The High Deductible Health Plan (HDHP) is a PPO plan. It provides you freedom to go in and out of Anthem’s Prudent Buyer PPO provider network. To supplement this medical plan, you will have an optional tax-exempt account known as a Health Savings Account (HSA), in which you can accumulate savings to pay medical expenses that make up your deductible and additional qualified health care costs.

The Anthem HMO Plan offers comprehensive coverage at a low out-of-pocket cost to you and your family. The primary objective of an HMO is to have only one physician coordinate a patient’s care through the HMO network of physicians, medical offices and hospitals. Under this plan, you MUST choose a Primary Care Physician (PCP) and a medical group. If you do not, Anthem will assign a PCP and medical group to you. Current HMO Members will be receiving new member ID numbers and new ID cards will be sent to your home.
